Please complete this form if you are interested in becoming a volunteer at Lord of the Streets. Once you complete the form, please click the "Submit" button at the bottom. All questions or blanks with a "*" are required information.
Volunteers at Lord of the Streets must be 18 years of age or older. When we receive your interest form, we will contact you. 
​You will be asked to show a photo ID upon arriving at LOTS for your first time volunteering.

    Have you ever been convicted of a criminal offense more serious than a traffic citation? (This will not necessarily disqualify you from volunteering at Lord of the Streets).
