Volunteers at Lord of the Streets must be 18 years of age or older. When we receive your interest form, we will contact you.

    VOLUNTEER STATEMENT OF UNDERSTANDING, AUTHORIZATION, RELEASE AND INDEMNITY 
    I, the undersigned (volunteer), am at least 18 years of age and am fully competent to make this Statement of Understanding, Authorization, Release and Indemnity, which I have read and understand. 
    I understand the information I have provided may be verified and permit Lord of the Streets to inquire of others concerning my suitability as a volunteer. 
    In the course of volunteering, I will deal with confidential information and acknowledge that I am bound under the regulations of the Texas Open Records Act to maintain the confidentiality of any client information that I may obtain and that I am subject to penalties under the provisions of this act. (Note: Any professional providing pro-bono mental health or social work services are also bound by license requirements.) Further, I understand that violating client confidentiality will result in my immediate dismissal as a volunteer. Certain volunteer positions may require a background check before volunteering. 
    I understand that my relationship with Lord of the Streets is an at will arrangement and may be terminated at any time, without cause, by either Lord of the Streets or by me.
    I understand that my work with Lord of the Streets is that of a volunteer only, and that I am not entitled to any compensation for performance of duties as a volunteer and that I am not entitled to any employment benefits, of any kind or character, and am not covered by any Worker's Compensation program.
    I am familiar with Lord of the Streets with which I will be volunteering and understand that I likely will be working with clients and/or other volunteers who may be homeless, convicted criminals, sexual offenders, or individuals with mental health or substance dependency issues. In return for the opportunity to serve as a volunteer with Lord of the Streets, I hereby forever release, acquit and discharge Lord of the Streets and its officers, directors, trustees, agents, employees, representatives, affiliates, successors and assigns (collectively the Released and Indemnified Parties) from any and all claims, demands and causes of action of any and every kind or nature, including those caused in whole or in part by the negligence of any of the Released and Indemnified Parties, which I may now or in the future have against any or all of the Released and Indemnified Parties and that arise in whole or in part as a result of my involvement with Lord of the Streets.  
    I also understand and agree that Lord of the Streets assumes no liability for accidents or acts of negligence or gross negligence by anyone, including Releases and Indemnified Parties. 
    I further agree to fully indemnify and defend any of the Released and Indemnified Parties against any and all claims, demands or causes of action of any and every kind or nature (including attorney's fees and other defense costs), including those caused in whole or in part by the negligence of any or all of the Released and Indemnified Parties, which directly or indirectly relate to personal injuries or property damages sustained by me and that arise in whole or in part as a result of my involvement with Lord of the Streets. 
    I EXPRESSLY AGREE AND ACKNOWLEDGE THAT I HAVE CAREFULLY READ THIS AGREEMENT, ITS CONTENTS, UNDERSTAND IT AND VOLUNTARILY SIGN IT. I further agree that no oral representation, statements or inducements apart from the foregoing written agreement have been made. I affirm that all the information contained in this application is true and correct and that there are no material omissions of fact concerning the information herein. 
    If any provision of this Statement is determined to be unenforceable, all other provisions shall remain in full force and effect.
    I understand that I cannot serve as a volunteer until this Statement has been signed.
    I agree and accept all of the provisions of the foregoing Statement of Understanding, Authorization, Release and Indemnity.
